State Requirements to Work
The Following Forms Must be Completed to Register with Absolute Home Care

  • I-9 Form with Proper Original Identification.
  • Green Card, Work Permit, Picture ID, Social Security Card, Proof of U.S Residency, etc.
  • FDLE Back-Ground Check (Fill Out Forms and Submit $15.00)
  • Two written References from former employer or school
  • Home Health Aide Certificate (Original 75 hrs or 40 hrs certificate and the 35 hrs Addendum)
  • Aids/OSHA/HIV Certificate (Original 4.0 Hours)
  • In Service Certificates (12-CEU's Including OSHA/HIV/Domestic Violence) CPR Card Must be updated yearly
  • A Physical with a PPD or chest X-Ray which is not more than 6 months old.
Everything must be up to date and complete in order to turn in your application.
